First Visit Information

Thank you for choosing Experience Momentum for your Physical Therapy, Massage Therapy, or Nutritional care. Below you will find the necessary information you will need on your first visit with us.

Please arrive at our facility at least 15 minutes prior to your scheduled appointment to fill out required paperwork. You also have the option of requesting required forms (except for Women's Health and Pelvic Floor), filling them out and bringing them with you to your initial visit. To do this, please contact us at least 48 hours prior to your first appointment. We have changing rooms, showers, and lockers for your convenience.

  • Parking at Experience Momentum in Lynnwood is free and located onsite. We do offer overflow parking just west of our facility near the espresso stand. We recommend arriving 20-30 minutes early if you have an appointment after 5:00pm.
  • Parking at Experience Momentum in Seattle is limited onsite, but is available on the street, for hourly purchase at the Brooks Headquarters or at additional pay lots within 1-2 blocks of the facility. Street parking is both 1-Hour and 2-Hour. Please pay attention to signage. We recommend arriving 20-30 minutes prior to your appointment if you are not used to city parking or the Fremont area.

Physical Therapy New Patients:

All initial visits last up to 53 minutes in length. This visit will consist of an interview and an evaluation of your concerns and needs. Depending on your needs, your Physical Therapist will provide treatment interventions, create a home exercise program, and/or prescribe additional treatments by other providers. However, your Physical Therapist may also decide to provide treatment and your home program at the second visit. Please note that home programs are customized to your condition and are built from scratch for each condition.

  • Please wear active clothing so you can move easily.
  • Payment information: we accept various forms of payment for your care whether it be cash, check, credit or debit. Additionally, if you qualify, we accept insurance. All deductible, Co-Pay, and additional payments are expected at the time of service. 
  • Referral and/or prescriptions may be required by your insurance to receive care from a Physical Therapist in the State of Washington. Please check in with us whether you need or require one. Yes, we agree that this is annoying and unnecessary.
  • Insurance information: we accept most major insurance, however do not accept Medicare or Medicaid. As a complimentary service, we will call your insurance and learn about your coverage. Please note, it is your responsibility to know your insurance coverage and cover any remaining costs that are not paid by the insurance. Please see our Costs & Insurance page to learn more about how we interact with your insurance company and to know what you we will be held responsible for.

Women's Health Physical Therapy New Patients:

All initial visits last up to 53 minutes in length. This visit will consist of an interview and an evaluation of your concerns and needs. Your Women's Health Physical Therapist will determine whether you require specific interventions during the initial or follow-up visits. 

  • Intake forms are specialized and different from our Physical Therapy intake forms. Women's Health Intake forms are provided at time of service. Please present 20-30 minutes prior to your initial appointment. 
  • Referral and/or prescription is required for Women's Health care. Please talk with your PCP or OBGYN to obtain this. 
  • Insurance information: insurance is billed the same as we do Physical Therapy. Reference above.

Massage Therapy New Patients:

All initial visits last up to 53 minutes in length. This visit will consist of an interview and an evaluation of your concerns and needs, along with Massage Therapist soft tissue interventions.

  • Intake forms will be provided upon arrival or you may request and print off the intake forms prior to the visit to save time. This information is required by most insurance companies to submit for reimbursement and also assists our providers in determining your course of care.
  • Payment information: we accept various forms of payment for your care whether it be cash, check, credit or debit. Additionally, if you qualify, we accept insurance. All deductible, Co-Pay, and additional payments are expected at the time of service. 
  • Referral and/or prescriptions may be required by your insurance to receive care from a Massage Therapist. Please check in with us whether you need or require one.
  • Insurance information: we accept most major insurance, however do not accept Medicare or Medicaid. As a complimentary service, we will call your insurance and learn about your coverage. Please note, it is your responsibility to know your insurance coverage and cover any remaining costs that are not paid by the insurance.   • Clinical Massage vs. Relaxation Massage: If you are receiving a clinical massage as determined by a Medical, Osteopathic, Physical Therapy, or other doctor, your care does qualify to be reimbursed for insurance coverage. However, if you are receiving a massage for relaxation purposes, we cannot submit for reimbursement. Please see our wellness plan for more cash pay options.

Nutritional Counseling New Patients:

All initial visits last up to 53 minutes in length, but will depend on the goal of the appointment. This visit will consist of an interview and an evaluation of your concerns and needs. No meal plans or written programming will be provided at your first visit unless otherwise specified. All plans and programming are built custom to your specific needs. 

  • Intake forms will be provided upon arrival or you may request and print off the intake forms prior to the visit to save time. This information is required by most insurance carriers to submit for reimbursement and also assists our providers in determining your course of care.
  • Payment information: we accept various forms of payment for your care whether it be cash, check, credit or debit. Additionally, if your plan qualifies, we accept insurance for nutritional counseling. All payments are expected at the time of service.
  • Referral and/or prescriptions may be required by your insurance to receive care from a Registered Dietitian. Please check in with us whether you need or require one.
  • Insurance information: we accept most major insurance, however do not accept Medicare or Medicaid. As a complimentary service, we will call your insurance and learn about your coverage. Please note, it is your responsibility to know your insurance coverage and cover any remaining costs that are not paid by the insurance.
